Job Title: Chief Medical Officer                                                                                           Location: Stockton, CA

The Chief Medical Officer is responsible for planning, directing, and evaluating the medical care component of the Community Medical Centers (CMC) delivery system, and for providing direct primary physician services to the program's target population.  The Chief Medical Officer reports to the Chief Executive Officer.  MINIMUM REQUIREMENTS: Must be licensed as a physician in the State of California, must have three to five yearsí experience in medical administration in a community health center or similar setting, knowledge and/or experience in working with populations, and current certification in basic cardiopulmonary resuscitation. The Licensed Clinical Social Worker is responsible for the devel≠opment, implementation and evaluation of case management services and counseling, and facilitation of support groups for clients with health related issues.  The licensed Clinical Social Worker reports to the Medical Director. Job Title: Facilities Manager                                                                                           Location: Stockton, CA

The Facilities Manager position handles medium to high complexity facilities issues, requests and projects throughout all CMC facilities.   This position uses critical thinking, problem solving, project management and high execution skills, to ensure facilities requests are completed on time, on budget and accurately, over the entire life of the project, from conception to completion.  This position reviews and analyzes facilities requests, discerns customer needs, proposes solid and financially prudent solutions, provides continuous quality communication to all affected, oversees execution and vendor/department performance, and works collaboratively with internal and external departments and vendors. Minimum requirements: At least 5 years' experience in facilities operations, basic knowledge of small hand tool operation, able to read and understand equipment operation manuals and schematics, working knowledge of basic safety protocol practices, including electrical and mechancial, proven knowledge of basic safe body mechanism practices, working knowledge of basic building code and Cal OSHA regulations preferred, training in HVAC operation, service and maintenance, plumbing systems, fire alarm system requirements, building automation systems, carpentry and finish work in a commercial building environment, and certified Healthcare facility manager certification preferred. The Clinical Site Manager is responsible for the delivery of optimum patient care at the clinic site. The position directs day-to-day operations, supervises nursing staff and medical assistant activities to ensure quality of care, patient flow and compliance with regulations and governing agencies. This position is responsible for staff development and performance. The position reports to and is under the general direction of the Chief Operations Officer, and collaborates to facilitate the provision of quality nursing care. Minimum qualifications: Bachelorís degree in Nursing with at least three (3) years progressively responsible management and supervisory experience with specific knowledge, experience, and demonstrated ability in the areas of management, staff development, organizational evaluation, professional staff development and leadership skills in an ambulatory medical care setting with preference given for experience in Community Health Centers,  knowledge and understanding of customs, beliefs, and needs of consumer populations served, ability to work independently with minimum supervision, written and verbal communication skills in English commensurate with college level education, with preference given for written and verbal communication skills in Spanish, current CPR card from the American Heart Association. Title: EHR Analyst I

Responsible for the implementation, optimization, and training of clinical information systems that assist clinicians in the delivery of patient care. Participates as a member of the EHR Steering and Change Management Committee in representing the needs and requirements of the physician community and serves as an advocate of management in promoting the use of information technology in the clinical setting. Title: Outreach and Enrollment Counselor                             
Location: Vacaville and Dixon, CA

This position will perform outreach and enrollment assistance activities that facilitate enrollment of eligible health center patients and service area residents into affordable health insurance coverage through the Health Insurance Marketplaces, Medicaid and The Childrenís Health Insurance Program/Healthy Families.  The Outreach and Enrollment Counselor must comply with and successfully complete all required and applicable federal and state consumer assistance training.  This position will assist in all parts of the insurance enrollment process such as educating consumers about affordable coverage options, setting up a profile in the portal, filing affordability assistance information, receiving an eligibility determination, and/or enrolling in affordable health insurance.  The Outreach and Enrollment Counselor must demonstrate and maintain expertise in: eligibility and enrollment rules and procedures; the range of qualified health plan options and insurance affordability programs; the needs of underserved and vulnerable populations; and privacy and security standards.  At a minimum this position will perform the following required duties: conduct public education activities to raise awareness about coverage options under Medicaid, CHIP and the Marketplace; help individuals understand and access affordability options; provide information and assistance in a fair, accurate and impartial manner; provide information and assistance in a manner that is culturally and linguistically appropriate to diverse communities and accessible to individuals with disabilities; and provide referral to any applicable office of health insurance consumer assistance or ombudsman established under Section 2793 of the PHS Act to address consumer grievances, complaints, or questions about their health plan, coverage, or a determination.  Priority will be given to veterans and applicants that possess the ability to operate the technology necessary to ensure that no consumer Personally Identifiable Information is compromised.  Minimum requirements include one year experience working in a health program providing education regarding health insurance coverage, valid driver Licence, auto insurance, and personal transportation.
