Job Title: Licensed Clinical Social Worker                                                                       Location: San Joaquin County
FLSA Status: Exempt

 

The Licensed Clinical Social Worker is responsible for the devel­opment, implementation and evaluation of case management services and counseling, and facilitation of support groups for clients with health related issues.  The licensed Clinical Social Worker reports to the Medical Director. Munimum requirements: Master's Degree from an accredited school of social work, one year of post MSW social casework, possession of a valid California license as a Clinical Social Worker, experience with crisis intervention services and use of case management techniques, public speaking ability, experience counseling individuals and families, valid California driver's license, proof of auto insurance, and personal transportation. Job Title: Fund Development Director                                                                                  Location: Stockton, CA
FLSA Status: Exempt

The Fund Development Director is responsible for the overall planning, development and management of the agency’s fund and grant development partnerships and activities in support of Community Medical Centers’ strategic initiatives. The Fund Development Director reports to the Chief Executive Officer.   Minimum requirements: Master's Degree with two years related management experience in grant and fund development within a nonprofit organization or   Bachelors Degree and a minimum of three (3) years related experience in the areas of nonprofit fundraising, grant writing, marketing, communications, community affairs and management with a successful fund development track record, Excellent written and verbal communication skills in English commensurate with college level education, valid California Driver's License, proof of insurance, and personal transportation. Job Title: Patient Navigation Center Supervisor                                                               Location: TBD
FLSA Status: Exempt

The Patient Navigation Center Supervisor is responsible for the day to day management of the CMC Patient Navigation Center to achieve timely, accurate and complete patient referrals to external providers for care, treatment and services.  This position also arranges for appropriate hospital and Emergency Department follow-up care with the PCP. Strategic planning for the improvement of efficiency and effectiveness of the Patient Navigation Center’s activities is carried out to enhance customer service and patient-centered care. The Patient Navigation Center Supervisor reports to Chief Operating Officer. Title: Application Support Specialist I

The Application Support Specialist provides assistance related to the support of practice management, electronic health record and other clinical informatics applications and software. Responsible for system updates and on-going staff training for health center and administration monitors data entry and system reports to identify training needs. This position assists in the formulation of procedures and best practices for end-users. Minimum requirements include: Ability to assume sole and independent responsibility for various assigned projects and tasks, ability to exercise discretion and independent judgment with respect to significant matters, strong interpersonal skills including the ability to work well with people with different backgrounds, Associates, bachelor's dregree, or completion of certification program with 2 years of healthcare experience or 3 years experience in the healthcare field,prior experience with set up and configuration of EPM/EHR or other clinical informatics applications preferred, knowledge of clinical workflows and billing processes in ambulatory care setting, strong organizational skills and attention to detail, ability to work on several projects simultaneously, excellent written and verbal communication skills, strong analytical skills with the ability to focus on detail, ability to complete projects with tight deadlines and time constraints, knowledge of HIPPA and related privacy/confidentiality laws and regulations, solid understanding of the implementation process, proficient in the following Microsoft Office applications, including Outlook, Excel,  Word, PowerPoint, Access, and Project and an understanding of Visio, ability to travel often, valid California Driver's license, proof of insurance and personal transportation.
