Here is some
important information about your clinic appointments and payment for
services:
Appointment
Information
Appointments ~ we
operate on an appointment system. Appointments may be made by calling
your clinic appointment desk. Same day appointments are available for
patients with urgent conditions. If you are unable to keep your
appointment please call to reschedule or cancel.
A Member Card ~ will be
issued to you with your family number. Use this number when calling to
schedule appointments.
After Hours Assistance ~
is available to patients by calling your clinic. If you require
emergency services call 911. Our preferred hospital is San Joaquin
General Hospital.
Prescriptions ~ may be
filled at Channel Medical Pharmacy, 701 E. Channel in Stockton, or at a
community pharmacy of your choice. For refills, contact the
pharmacy that filled your prescription.
Payment Information
Visa and MasterCard
Accepted
A $30.00 Deposit is Required
at Each Visit ~ as payment toward your balance, depending on
your insurance coverage.
Medi-Cal, Blue Cross, Health
Plan of San Joaquin, Partnership Health Plan & Healthy Families Clients
~ please bring your current member card to every appointment.
Discounts are Available Based
on Income for Patients Without Insurance or Medi-Cal ~ please
bring verification of income on your first visit to determine
eligibility (tax return or pay stubs).
Billing ~
statements are mailed monthly and payments must be paid monthly.

No Cost Services ~ many
services are available at no cost to qualifying families (based on
income). Please ask the receptionist about eligibility
requirements for programs that provide well-baby care and
immunizations, childhood physical exams, family planning services and
confidential HIV testing.
