Health Education and Community Outreach
Health Education Health Education group and community
strategies improve health skills, behavior, and attitudes. The purpose of
health education is to create a positive influence on individuals and
community health behaviors. By focusing on prevention, health education
can reduce the costs spent on medical treatment and increase patient
compliance to medical treatment. "Enhancing the quality
of life for all people" Community Medical Centers, Inc. (CMC) offers a
Variety of Health Education programs in San Joaquin, Solano and Yolo
Counties. Current Collaborators include Stockton Unified School District,
Health Plan of San Joaquin, Kaiser, Solano County and Yolo County.
Clinic-Based Education CMC has developed a preventive health
program geared towards patient self-management. CMC provides clinic-based
health and nutrition education that includes one-to-one education and
classes in diabetes, cholesterol, obesity, hyperlipidemia, and
hypertension. Services provided by Health Educators are geared towards
initiating patient behavior change. All Health Education is provided free
of charge. Community Outreach Staff is available to
participate in Health Fairs, Community Presentations and Outreach events
