The Health Education Department strives to help patients gain confidence about their health condition and empowers them to make changes to improve their overall health.

Clinic-Based Health Education

• Information, guidance, and support with management and prevention of chronic conditions, including obesity, diabetes, high blood pressure, high cholesterol, and asthma
• Group classes available in various topics in-person and on Zoom!
• Referral to community services & resources
• Appointments are FREE to all CMC patients
