As the Health Education and comunity outreach team we seek to prevent illness, promote wellness, and support individuals and families in their management of chronic conditions.

Clinic-Based Health Education

  • Information, guidance, and support with management and prevention of chronic conditions, including obesity, diabetes, high blood pressure, high cholesterol, and asthma
  • One-on-one sessions and classes
  • Referral to community services & resources
  • Appointments are FREE to all CMC patients

Community-Based Outreach, Education and Initiatives

  • Participation in special events, and health & resource fairs
  • Educational presentations throughout CMC service areas
  • Multiple collaborations advance and support broader community health goals
  • Smoking cessation education targeting high school students [Stockton Unified School District]
  • Outreach, education, Medi-Cal enrollment and renewal assistance targeting uninsured and Medi-Cal eligible residents [San Joaquin County Human Services Agency]
  • Obesity prevention and health insurance utilization assistance targeting homeless and addicted individuals in San Joaquin County [Gospel Center Rescue Mission]
  • Case management services targeting individuals with HIV & AIDS [Solano County Public Health]

  • Information about free and low-cost health insurance options that meet your needs
  • On-site Certified Application Counselors at most CMC locations
  • FREE assistance with Medi-Cal and Covered CA health plan enrollment and renewal
