Job Title: Chief Nursing Officer (CNO)
Location: Stockton, CA
FLSA Status: Exempt

As a member of the CMC’s executive team, the Chief Nursing Officer supports and facilitates an interdisciplinary team approach to the delivery of care to patients, families and the community. This includes creating an environment in which collaboration is valued and excellence in clinical care, education, and innovation is promoted and achieved. The Chief Nursing Officer reports to the Chief Executive Officer. Master's degree with 3 years of experience, Bachelor’s degree with 5 years of experience, or AA degree with 7 years of experience.  Experience should reflect progressive responsibility in nursing operation management and experience in a management level leadership role. Management and Leadership Experience at an executive level is preferred. Licensed as a Registered Nurse in the State of California. Job Title: Chief Information Officer (CIO)
Location: Stockton, CA

As a member of the CMC’s executive team, the Chief Information Officer (CIO) provides corporate leadership, vision, and direction for organization technologies in support of the organization's strategic plan, business objectives and requirements. The CIO is responsible for the overall architecture, design, development, and maintenance of the organization’s technical hardware and software infrastructure with a focus on hardware and software engineering, data architecture, network design, cloud computing, mobile device capabilities, software interfaces, middleware software and systems integration, and driving the overall technical transformation aligned with the IT and Healthcare industries. The CIO also participates in due diligence reviews of potential acquisitions. The CIO supervises IT management and serves as a strong consultant to the various departments, especially in areas such as marketing, medical service and staff support, where the intersection of existing and emerging technologies can impact quality of care, top-line revenue and improved margins. The CIO reports to the Chief Executive Officer. Bachelor’s degree in Computer Science, Information Systems or other technology-related field.  Master’s degree is preferred. 5 or more years of management experience with computer information systems including security, networking, server, software application management and support within a healthcare setting. Experience in a health care organization preferred. At least three (3) years of high level systems management in a leadership capacity including network infrastructure, telecommunications, and systems analysis. Able to work flexible hours as needed. Job Title: Controller

The Controller is responsible for performing finance functions related to the practice management and accounting systems. This position is responsible for input and output, promoting and improving systems, providing proper audit trails, controlling record protection and other related work. Job Title: Director of Pharmacy Services

The Director of Pharmacy is responsible for the supervision and coordination of functions related to the pharmacy, including the evaluation, selection, procurement and distribution of drugs, patient counseling and professional consultation, in a Patient Centered Health Home context. Job Title: Physical Therapist Part time or Full time

The Physical Therapist is responsible for providing general physical therapy care to CMC’s target population as part of an integrated care team model.  The role of the Physical Therapist is to work as part of the Health Care Team to help restore patient's function, alleviate pain, and prevent disabilities by planning and administering medically prescribed physical therapy. The Physical Therapist reports to the Chief Medical Office and/or Specialty Director. Job Title: Substance Use Counselor
Location: Stockton, CA
FLSA Status: Exempt

 

The primary focus of the Substance Use Counselor is to use a patient centered approach and effective communication to assist patients to cope with the substance abuse and dependence issues and practical arrangements related to the patient’s illness. Effective outreach and engagement strategies that establish trust and overcome barriers resulting from isolation, stigma, and the symptoms of mental health and/or substance use problems will be utilized. A sustained support model to meet basic needs and respond to individual preferences as well as trauma-informed services that restore hope will be utilized. The Substance Use Counselor is a member of the healthcare team providing assessments, coordination, treatment planning, information and referral to community resources and other social work services. The primary goal and offered individualized and group intensive substance use services using a multidisciplinary approach, including, but not limited to: Mental health and substance use assessment; health system navigation; housing and benefits advocacy; health coaching, referrals and brief treatment, pharmacy consultation and supportive counseling.  The Substance Use Counselor reports to the Director of Behavioral Health.  Minimum requirements: High School Diploma or GED,  one year of experience as a Counselor or Social Worker in a treatment program facility is required, Alcohol and Drug Counselor Certificate issued by the State of California, must possess knowledge of chemical dependency, substance use and personality dynamics of the substance user, excellent Microsoft Office Program skills, current CPR Certificate or ability to obtain a CPR Certificate, valid CA driver license, proof of auto insurance, and personal transportation. Job Title: Patient Services Representative

Job Title: Patient Services Navigator Team Leader
Location: Stockton, CA

The Patient Services Navigator Team Leader is responsible for leading the Patient Service Navigators in regards to patients who are in transition of care after being discharged from the hospital or receiving care in the emergency department. The overall goal is to reduce the risk of hospital readmissions and to avoid unnecessary emergency department visits by navigating the patient through the healthcare system.  The Patient Services Navigator Team Leader reports to the Patient Navigation Center Supervisor or designee.
