The Recovery Center is an out-patient center providing services to individuals with substance use and/or mild to moderate behavioral care needs. Patients are referred to the Recovery Center through a variety of sources that include; referral given by CMC Behavioral Health Clinicians, Medical Providers, Case Managers, external providers, agencies or self-referral. The center provides a full assessment for individuals with mild to moderate substance and mental health care needs. Patients may be referred to case management, recovery counseling, and/or medically assisted treatment. The center will also provide sobering services. Patients that receive services may present under the influence of alcohol and may need medical assistance by Recovery Center employees. Job Title: Substance Use Counselor II

Job Title: Survivor Advocate Community Health Worker

The Survivor Advocate Community Health Worker (CHW) will provide outreach, counseling, navigational, and case management services to patients who are victims and survivors of human trafficking and/or intimate partner violence.  Job Title:  Patient Services Navigator F/T-Regular and F/T-Temporary positions available
Location: Stockton, CA
FLSA Status: Non-Exempt   

 

The Patient Services Navigator (PSN) is responsible for assisting patients who are in transition after being discharged from the hospital or receiving care in the emergency department. The overall goal is to reduce the risk of hospital readmission and to avoid unnecessary emergency department visits by navigating the patient through the healthcare system.  The PSN reports to the Patient Navigation Center Supervisor. Minimum requirements: Minimum of 1 year of experience in an ambulatory setting delivering care as a Medical Assistant, Health Educator, Nurse or similar, knowledge and use of electronic health record systems, electronic population management systems and Microsoft office is desired, bilingual English/Spanish required, valid California driver's license, proof of auto insurance, and personal transportation. To apply, please click on the link below.
