About

The organization that is now Community Medical Centers started as a volunteer effort in the late 1960s because people in this community care about each other. We remain deeply rooted in the community and are dedicated to improving the health and quality of life for its members. Every day, our staff provides vital care to men, women and children, regardless of ability to pay.

We are a growing network of neighborhood health centers serving over 100,000 patients in San Joaquin, Solano and Yolo Counties, making us one of the area’s largest safety net providers. We offer integrated medical, dental, behavioral health, and supportive services, all using a model that puts the patient at the center of the care team.

We are working together to improve health and well-being in our communities.

 

Community Medical Centers, Inc. is a Federally Qualified Health Center and is certified by the Joint Commission as a Primary Care Medical Home (PCMH). It is a Health Center Program grantee under 42 U.S.C. 254b, and has Public Health Service (PHS) deemed status under 42 U.S.C. 233(g)-(n) with respect to certain health or health-related claims, including medical malpractice claims, for itself and its covered individuals.
