families in their management of chronic conditions.
Clinic-Based Health Education
- Information, guidance, and support with management and prevention of
chronic conditions, including obesity, diabetes, high blood pressure, high
cholesterol, and asthma
- One-on-one sessions and classes
- Referral to community services & resources
- Appointments are FREE to all CMC patients
